NewStepNewStep Home Care

Hospital to Home

The days after coming home decide the months that follow.

NewStep provides non-clinical practical support and companionship for the window between leaving a ward and living confidently at home again — working alongside the health and care services already involved, never in place of them.

A five-stage approach, followed every time.

  1. 01

    Enquiry

A family member, discharge coordinator, social worker or the client themselves contacts us. We respond the same working day where possible.

  2. 02

    Initial home visit

A support assessment at home: the routine, the household and the practical help that would make returning easier, alongside any services already involved.

  3. 03

    Agreed support plan

We confirm in writing what we will help with, the support hours, when we will visit and what it costs — and we are clear about anything outside our non-regulated remit.

  4. 04

    Settling home

We prepare the home, meet the client on arrival where arranged, and provide close practical support and company through the first days.

  5. 05

    Ongoing review

We review regularly and step support hours up or down as confidence and independence return.

What the support includes

  • Preparing the home: heating on, food in and walkways clear
  • Meeting the client at home on the day of return, where arranged
  • Meal preparation, shopping and keeping the fridge stocked
  • Light housekeeping, laundry and everyday household jobs
  • Company and reassurance during the first unsettled weeks
  • Accompanying follow-up appointments and therapy sessions
  • Letting the family know promptly if something seems wrong
  • Reducing support hours as confidence and independence return

What we do not provide

  • No personal care such as washing, bathing, dressing or toileting
  • No administering or managing of medicines
  • No nursing, clinical tasks, treatment or health monitoring
  • No support that requires registration with the Care Quality Commission

For professionals

Arranging practical support

Discharge coordinators, social workers, therapists and community teams can contact us by email or through our enquiry form to arrange non-clinical practical support and companionship alongside any regulated care or health service you are commissioning.

Please include the expected date of return home, the practical help needed, who else is involved and the contact details of the responsible professional. We respond the same working day wherever possible and confirm in writing what we can provide, from when, and anything that falls outside our non-regulated remit.

NewStep is not registered with the Care Quality Commission. Personal care, medication support, nursing, therapy and treatment remain with the regulated provider or health service involved; our role is the practical and social support around them.

Availability and any commissioning routes will be published here once confirmed by the business. No local authority or NHS contract is claimed.
